Public Review: Housing-Focused Outreach Manual & Complaint and Grievance Policy

PUBLIC REVIEW PERIOD: Monday, August 31 to Monday, September 14

Housing for Health (H4H) is continuing to strengthen and clarify policies, procedures, and guidance for the Housing for Health Partnership (H4HP) System of Care. As part of this work, H4H Division staff are developing new and updated materials to promote consistent, effective, and person-centered practices across the system. Input from partners, providers, people with lived experience, and other stakeholders is an important part of this process.

H4H is currently seeking feedback on two draft documents: the Housing-Focused Outreach Manual and the Complaint and Grievance Policy.

Connector Collaborative: Queue Case Conferencing & Housing Problem Solving

When: Every 4th Thursday of the month, 1:00 PM - 3:00 PM

This session: Thursday, September 24, 2026, 1:00 PM - 3:00 PM

 

Join us for our monthly Connector Collaborative meeting, which combines Virtual Case Conferencing and Housing Problem Solving into one collaborative session.

These meetings provide an opportunity for Connectors to work together to:

  • Address barriers for participants with extended stays on the Housing Queue.
  • Review progress on limited housing referrals.
  • Explore housing problem-solving and self-resolution strategies.
  • Coordinate services and resources across agencies.
  • Identify opportunities, troubleshoot barriers, and assign follow-up actions.

Discussion will include:

  • Follow-up Housing Queue cases.
  • Newly identified Housing Queue cases.
  • Connector-nominated housing problem-solving cases.

Before the meeting:

  • Housing for Health (H4H) will distribute the Housing Queue list and identify participants for discussion.
  • Please review the identified cases, prepare participant updates, and submit any additional housing problem-solving cases to Adriana by the Monday prior to the meeting, include the participant's HMIS UI#.
  • If you are unable to attend, please notify Adriana in advance and email your case updates.

We look forward to collaborating with you to reduce barriers, identify housing opportunities, and support positive outcomes for participants.

Housing for Health (H4H) is holding its monthly HMIS Office Hours, which is scheduled for the fourth Monday of every month. During this meeting, all HMIS user are encouraged to attend to learn about recent and upcoming changes to the system or HMIS polices, as well as to ask Housing for Health and Bitfocus staff about an issues, questions, or concerns they have with HMIS.

Meeting Location: Santa Cruz County Sheriff’s Office Community Room, 5200 Soquel Ave. Santa Cruz, CA 95062

 

This training is designed for those that have been invited to serve as Coordinated Entry Connectors. It will cover the Coordinated Entry process, the tools, including the Housing Needs Assessment (HNA) and the Housing Action Plan (HAP). Including interactive time to test the tools and approach.

Housing for Health (H4H) is holding its quarterly all-day, in-person General HMIS training. This training is for new and prospective HMIS license holders and covers the basics of how to access, navigate, and enter client data into the HMIS system. The training will be facilitated by H4H’s Jessica Scheiner and Loren White.
